1. Full name, address, and nationality of the Applicant.
Documents:
1. A notarized Power of Attorney from the Applicant (a copy is acceptable at the time of filing but the original must be submitted within three months from the filing date).
2. A certified copy of the Certificate of the Protection of the Appellation of Origin granted in the origin country or a Document issued by the Competent Authority of the origin country attesting to the right of the Applicant to use the Appellation of Origin (required at filing).
